Privacy Protection Practices
Oasis Fertility Centre wishes to provide its patients the best quality health care. To do so we acquire and store certain personal information about our patients. We therefore wish to keep you informed about our privacy protection practices. Privacy Protection is a requirement of both the Health Information Act of Alberta and the Personal Information Protection and Electronic Documents Act (PIPEDA) enacted by the Federal Government of Canada in 2014.
Protecting personal and private information is not a new concept to us as health care providers. Ensuring patient confidentiality has always been one of the highest priorities in our practice. Our staff takes this very seriously and we have a number of procedures in place to ensure that your personal information remains confidential.
What is personal information?
Personal information includes all details that identify you as an individual. These include, but may not be limited to, your name, age, date of birth, AHCIP number or identification number, marital status, number of dependents, your entire medical record, and any financial information we may require.
How is your personal information used?
Your personal information is used to provide you with the best possible medical care. We need your complete medical history, physical examination, and test results to determine the appropriate treatment. Other personal information may be used to allow us to better understand your needs and manage our own practice.
To whom may we release this information to?
Release of selected personal information may be necessary to the following people:
– Other physicians, specialized health care professionals or health care institutions whom Oasis Fertility refers you to for care.
– Your provincial insurance agency AHCIP or private insurer who requires specific information for payment purposes.
– Government regulatory authorities or government approved registries who require us to complete information by law or mandate.
We do not provide or sell personal information about you to any outside company for use in marketing and solicitation. No personal information about you or your dependents is released to a third party without permission (except for those parties noted above) or in a medical emergency situation. To ensure that Oasis is able to provide you with the best possible service, it is important that the personal information we use is accurate and up-to-date. Please keep us informed of changes in name, address, marital status, and your family physician. Should you become aware of errors in the information we have about you, please contact our receptionist immediately and we will make sure that your data is corrected.
By becoming a patient of Oasis Fertility, you are agreeing to allow your personal information to be used and disclosed in the manner outlined above. Should you wish to withdraw your consent you may do so at any time. This may however have an impact on the services which we can provide.
